Replacement Cards

If you misplace your membership card, you can request a new card. There is a $5 charge for printing a replacement card. Please allow two to three weeks for delivery. Please email the FONZ Membership Office to request proof of membership prior to your visit.

Please note that ordering a replacement card does not renew your membership. You may not order a replacement card if your membership has expired.
